We run many clinics for the management of chronic diseases and offer a wide variety of medical services.
Services include:
Long Term Condition Clinics
GPs, Pharmacists and Nurses
- Asthma
- CHD
- COPD
- Diabetes
- Hypertension
If you are diagnosed with any of the above conditions and it is appropriate for you to attend a clinic, you will be offered an appointment. Alternatively, if you feel that you require a review then please contact the Practice on 0208 548 7520 and an appointment will be arranged for you.
Family Planning
GPs and Nurses
- Contraception Advice
- Contraceptive Prescribing / Implant
All patients taking the contraceptive pill must see the nurse at least every 6 months for a contraceptive review. It is the patient’s responsibility to ensure that the appointment is arranged in advance so that they do not run out of their medication.
Social Prescribing
Social Prescriber
Social Prescribing takes into account that a person’s health is not only down to lifestyle choices and genetic makeup. It addresses other factors to make you feel better. Once referred to the Social prescriber, you will be put in touch with services and activities of your choice and at your pace.
Social Prescribing can help with but is not limited to:
- Bereavement-related anxiety
- Domestic violence
- Employability and employment support
- Healthy lifestyle support and weight reduction
- Housing
Please contact reception to book an appointment with our in-house Social Prescriber.
Cervical Screening
Nurses
Appointments can be booked by contacting reception. The smear test aims to detect changes in the cervix (neck of the womb) before they become dangerous so that they can be treated.
Practice screening intervals are:
- No routine screening for patients under the age of 25
- Patients aged 25 to 49 will be screened every 3 years
- Patients aged 50 to 64 will be screened every 5 years
(Unless otherwise advised by a clinician)
Patients who receive a letter of invitation to attend for a smear should make an appointment with the Practice Nurse.
